A Questionnaire in CXP is the component within a Survey where all your questions, content blocks, and themes are assembled. It allows you to assign specific configurations to different Entities, making your Surveys adaptable to various audiences or locations.
Each Survey can contain multiple Questionnaires, and each Entity can be linked to only one Questionnaire within a Survey. This flexibility enables personalized experiences for different audiences while maintaining centralized data aggregation across shared questions.
Before creating a Questionnaire, you must first create a Survey. Learn how to create a Survey here.

You can tailor the experience per Entity with features like:

⚠️ Note: Each Entity can only be assigned to one Questionnaire per Survey. Managing multiple Questionnaires can increase maintenance overhead, especially when changes are required across all Questionnaires.
How to Create a Questionnaire
Once you’ve created a Survey, navigate into it and click "New Questionnaire" to open the Questionnaire editor. The creation process includes the following steps:
1. Settings Step
Internal Name: Set an internal name for your Questionnaire (e.g., "Default Questionnaire").
Display Entity Address: Optionally show the Entity address under the Entity name.
Default Language: Select a fallback language. [Learn how languages work here.]
2. Welcome Step
Optional but recommended.
Choose a welcome message from your Content Library.
3. Content Step
Here’s where you build the core content of the Questionnaire:

Add Questions: Click "+ Content" to select questions from the Question Library.
Only questions not intended for the Submit Section appear here.
Each question can only be used once per Questionnaire.

Add Text or Sections:
Use "+ Section" to insert pre-defined sections.
Then add questions and text inside those sections via "+ Content".

Pagination Options:
All questions on one page
Split into multiple pages by defining the number of questions per page
Note: Pagination settings are not reflected in the editor preview.
4. Submit Step
Customize the closing section of your Questionnaire:
Add a section from your Content Library using "+ Section"

Add Submit-specific questions such as:
Contact Info (Name, Email, Phone, Room Number)
Data Privacy
Terms and Conditions
Push to Google
Add text contents (optional)
All added questions appear at the bottom in the order selected—except "Push to Google", which is always placed last.
5. Rules Step
This step allows configuration of question behavior:

Question Settings:

Mandatory Toggle: Applies to all question types
When question have the toogle "Impacts your KPIs and appears in widgets" enabled, the Email question is mandatory toogle is always enabled and is not possible to edit it.
- For surveys distributed via email or via the Survey API channel, the Email question is always pre-filled and hidden with the recipient's email address

Visibility Rules ("Show Question Only If"):
Available only for questions added in the "Content" Step.
Can be set by questions, entity attribute or visit data point.
You can add or delete multiple rules as needed. Note: All conditions are combined using AND logic—each condition must be met for the rule to apply.

6. Thank You Page Step
Select a Thank You Message from your Content Library
This step is mandatory
7. Email Invitation Step
Select an email invitation from your Content Library
Shows a preview with a "Write a Review" button

Optional: Embed the Overall Score question (if available)
Replaces the button with a question preview
- The preview renders the Overall Score question in the format configured for that question — numbers, icons, or stars
This is auto-disabled if the Overall Score question is removed
8. Theme Step
Choose a Theme from your Content Library
First theme in the list is selected by default
This step is mandatory
9. Scope Step
Select the Entities that should use this Questionnaire
Important: Ensure selected Entities are not already assigned another Questionnaire from the same Survey
Final Step
Click "Save and Close" to finalize and save your Questionnaire.
